How to Use This Template
This form records the details of any accident, injury, near-miss, or safety incident during your operations. It includes sections for date, time, and exact location, the injured or affected person's details, a factual description of what happened, witness details and statements, first aid or medical treatment given, whether emergency services were called, photographs of the scene, root cause analysis, and corrective actions to prevent recurrence.
Complete this form as soon as possible after any incident — memories fade and details get confused within hours. Be factual and specific: "Staff member sustained a burn to the left hand from the edge of the pizza oven while removing a pizza at approximately 14:30" is far more useful than "someone got burned." Photograph the scene, the injury (with consent), and any equipment involved.
Under RIDDOR regulations, certain workplace injuries must be reported to the HSE within specific timeframes — fractures, amputations, loss of consciousness, and injuries requiring hospital admission among them. Even for less serious incidents, a completed report protects you if a personal injury claim is made. Insurers routinely request incident reports. Keep them for at least six years (the limitation period for personal injury claims).
