How to Use This Template
This form collects structured feedback from customers at events and after bookings. It includes a simple satisfaction rating (1-5 stars), checkboxes for what they liked (food quality, presentation, value, speed, friendliness, professionalism), specific questions about the service, an open comment field, an optional email field for your mailing list, and a testimonial consent checkbox.
Print a batch of short versions for handing out at markets and events — keep these to one side of A5 so customers will actually fill them in. Send the longer version by email after private bookings and weddings, 2-3 days after the event while the experience is fresh. Include a direct link if you use an online form.
Customer feedback is free market research. Over a few weeks, you'll spot patterns — if everyone mentions speed, you might need to rethink your prep process. The email addresses build a mailing list for event announcements. Positive feedback becomes testimonials with consent. And critically, hearing about problems privately is infinitely better than discovering them in a public Google review.
