How to Use This Template
This form obtains written consent to use photographs of identifiable people for commercial purposes — your portfolio, website, social media, advertising, stock photography, or print materials. It includes the subject's details, event or shoot reference, a clear description of the images and intended usage, whether any payment or consideration is involved, the duration of consent, territorial scope, and a withdrawal clause compliant with UK GDPR.
Present this at every shoot or event where you want to use images commercially. Explain in plain language how the images will be used — be upfront about commercial applications. The form includes options to limit usage (portfolio only, social media only, all commercial purposes) so subjects can choose their comfort level. For minors under 18, a parent or guardian must sign.
Without a signed release, you cannot legally use identifiable portraits for commercial purposes in the UK. This applies even if you took the photo at a public event. Editorial use (news reporting) has different rules, but marketing, advertising, portfolio use, and stock photography all require explicit consent.
